Details you need to add someone to management or control
Have this information ready before you add someone to management or control.
When you apply to add someone to management or control of your registered establishment, we need to assess and approve them. For more information, go to How we check if someone is suitable for management or control.
Before you start your application, you can save time if you plan ahead. Talk to the person you want to add and let them know about the application. Some of the details you will need include their:
- legal name
- any other names they have used previously
- home address
- job title
- date and place of birth.
If any apply, you also need details of this person’s or their associates:
- convictions
- outstanding criminal charges
- government debts
- export-related infractions.
Associates include both personal and professional relationships to the person you want to add.
To add a person as a Request For Permit (RFP) declarant, you also need to provide their training documents. They must show the person is trained in the procedures of completing RFPs.
